• Personal information including name/age/ address etc.
• Determine medical condition of person being assessed (medical or non-medical)
• Determine if help is required with medications,
• Will you need caregiver help;
• Will you need post surgical help,
• Do you require help with shopping

We will require your Dr. information, if seeing one.

The initial meeting will take approximately one hour.
